I have spoken to other engineers at work today and we are in agreement that despite what the manual says we would not recommend running the engine while using the screw in adaptor. Yes some water might get to the impeller but your inpeller life would be greatly short'nd.
My dive club RIB has a 90 4 stroke and we never use this attatchment only muffs, but if I ever did use it I would not run the engine at all.
I cant imaging the maker telling you to do something that would damage the engine, they dont even mention muffs. if you dont run the engine how can you be sure the water is getting to all of the waterways?
OK, so theres still debate as to wether to run or not, so I am gioing to contact the head of the technical department at Barrus (Merc/Mariner importer) at work tomorrow.

any info from Barrus? It seems incredible that the flushing advise in the handbook supplied with Mercury / Mariner four strokes would damage the outboard.
Ok I couldn't get hold of the guy but I trawled through all our manuals and model update books and it would appear that Marine Power are happy for the engines to be run while using the flush adaptor. So do what you want but I prefer my impeller to have all blades attatched to it not running around my block!!!
Another thing I found with regard to using the adaptor was:
On models fitted with a separate thread next to the tell tale outlet when running engine make sure there is a good tell tale coming out. If there isn't stop engine imediately.
If you have to screw the adaptor in to the tell tale outlet itself your just going to have to hope enough water goes to the pump.
Insedently a guy came into work asking if he should run his engine while using said adaptor, we told him no!!
I still think muffs are the best way to flush your engine.
