My Tohatsu 50 hp 2 stroke was making a terrible noise when tilting up. It turned out to be low PTT fluid and air getting in the fluid.
you can see that after topping up the fluid and cycling about 12 times and re topping up to get rid of all the oil filled with air it seems and sounds hugely better. See below
