I have no idea what happened in that incident and fortunately, no one was hurt.
It's important to have a fire extinguisher onboard in an accessible location. Remember that over time, vibration will compact the contents of a powder unit so they should be given a tap on the deck and shaken before use. I've noticed this in a unit I carried in my vehicle previously. I grabbed it to tackle an engine fire I arrived at and sure enough - the powder had set in a block. Luckily I remembered to hit it off the ground first and as I shook it I could feel the block break up.
